Did you apply for an MC & DOT but never activated the authority within the 90 day time period? Use this application request form to reapply for your authority and you will get to keep your original MC and DOT numbers.

Please allow 3-5 weeks for activation. The ideal time to bind your policy is in two weeks. PLEASE BE ADVISED you only have 90 days to activate this authority with the FMCSA. Failure to do so will result in filing another reactivation application with the FMCSA and pay the $300 fees again.
