This form is used to notify NIDA student admin of a change to your preferred name or your legal name and/or gender and pronouns.
 
Your legal name determines what is recorded on your student enrolment and any government related documents (transcripts, fee help documents etc.). It can also alter other areas such as your email, your myNIDA account, ASIMUT, the headshot display, class lists, and your library record, etc.
 
Your preferred name will not be reflected in official documentation, but it can be edited on your email, your myNIDA account, ASIMUT, the headshot display, class lists, and your library record, etc.
 

Q1. Would you like to inform NIDA of a legal change of name? (This requires a digital copy of an identity document such as a drivers licence or marriage certificate showing the new legal name). *
Q2. Would you like to inform NIDA of a preferred name change? (This is in the case of no legal change of name. Your legal name will remain in the Student Portal due to government reporting requirements and will be printed on any graduation documents but your preferred name will be used in platforms such as emails, ASIMUT, MyNIDA etc.) *
