OAKLAND LADY PATRIOT BASKETBALL CAMP REGISTRATION JUNE 15-17, 2022

Welcome to our 2nd annual OHS Lady Patriot Basketball camp. The Camp will be June 15th-17th from 9am-12pm for rising 2nd-8th graders. Cost is $75 for all 3 days of camp. There are two payment options available. You can pay by check or cash at the door once you sign-in at camp. There are NOT any online payment options. This will include a t-shirt and a basketball. Please fill out this summer camp registration form if you plan on attending. Registrations are received online until June 10th, 2022 at 12:00am. You'll receive a confirmation message by email no later than June 11th. In case of an emergency, we can be contacted at 615-995-2750 during camp. We will take walk-up registrations, but it is not promised that you will recieve the correct size t-shirt.

I acknowledge and understand that in participation in this camp, the possibility exists that my child may sustain physical illness / injury (minimal, serious, and catastrophic) in connection with this camp. I indemnify and hold harmless Oakland High School and the Rutherford County schools system as well as its representatives from any claims for personal illness or injury that my child may sustain during camp.
