The only individuals who qualify to register at the member rate are owners, managers, licensed funeral directors, licensed embalmers and their spouses/significant others. Additionally, sponsors/vendors of the 2015 Annual meeting may register at the $375.00 rate.
Below list the names of those registering for the convention. List names exactly as they are to appear on name badges. For each person registered check each event that person will attend. If additional forms are needed, copied forms are permitted. Sign up for only those functions you plan to attend.
